Dancers, particularly those frequently performing movements requiring extreme ankle flexion, such as demi-pliés, are prone to a condition known as anterior ankle impingement. This condition is characterised by pain in the front of the ankle, resulting from the entrapment of bony and soft tissue structures during terminal dorsiflexion.


Causes

The causes of anterior ankle impingement are multifaceted and can include:


Poor biomechanics

Excessive foot pronation or improper technique and foot control, especially when landing in a demi-plié or during allegro, can contribute to the condition.


Previous ankle sprain

Scar tissue from a prior ankle injury can lead to impingement pain.


Genetics

In rare instances, a genetic predisposition can cause a bony blockage, leading to impingement.


Symptoms

The primary symptom of anterior ankle impingement is pain during movements that involve maximal dorsiflexion. Dancers may also experience a compressed or blocked sensation, resulting in reduced ankle mobility. Other signs include difficulty achieving full depth in pliés and struggling to lower heels when jumping.

Treatment

The treatment approach for anterior ankle impingement varies depending on the severity of the condition and may involve:


Activity modification

Reducing or altering activities that exacerbate the pain. A thorough history taken by your physiotherapist can help keep you active while minimising aggravating factors.


Taping

Joint mobilisation and manual therapy
